Procedure for submitting the CYSA Team Official Registration and Risk Management Disclosure (1628 form):

1. Go to the CYSA online Team Official Registration application (link is below).
2. Answer the questions online (you must have your driver’s license, social security number, or immigration visa number available).
3. In the Organization pull-down menu, be sure to select Almaden Valley YSL (0202) 
4. "Sign" electronically.  Although the form states that you must have a signed copy, you are only required to print the confirmation page.
5. Print the confirmation page and retain for your records or submit as required.
