Please fill out the form (see link below) and mail it along with a check made out to AVYSL to the tournament you are applying for - either the Almaden Champions Cup or Almaden Halloween Classic, 1321 Crossgates Lane, San Jose, CA  95120.  You no longer need to send the application to your District Tournament Coordinator.
