Please fill out the form (see link below) and mail it along with a check made out to AVYSL to the Almaden Halloween Classic, 1321 Crossgates Lane, San Jose, CA  95120.  You no longer need to send the application to your District Tournament Coordinator.

If you’d like confirmation that we received your application, include a self addressed and stamped envelope or postcard.  We will mail you back confirmation as soon as your application is processed.
